Dynamic Forms

The Dynamic Forms Module allows you to add as many forms to your website as you'd like.  You can choose the fields you'd like to include and  you can also include drop-down choices, checkbox choices or radio button choices.  You choose the email address that you'd like the form forwarded to and the auto reply message that you'd like visitors to receive when they submit a form to you.
